This product is designed as a centralized data management bridge and a decision support tool to healthcare providers of hospitals, LTC facilities, or an ACO. ACT will assist in ensuring a consistent continuum of care of therapies, medications, and general health monitoring across LTC and acute EHR systems. ACT also positions the hospital/clinic to maximize its 340-B funding through its unique ability to manage both Hospital and LTC data, as well as opens opportunities for funding through Meaningful Use and Transforming Clinical Practices initiatives.

Once connected, the core of the ACT product will effectively link a resident’s acute and LTC data. Then apply the data to decision support tools in order to identify clinical trends that will assist in maintaining the highest possible level of care while reducing the number of hospitalizations. ACT will provide actionable alerts around a number of healthcare identifiers which allows the facility/physician to take action by informing the physician of a potential medication/therapy change and/or triggering additional monitoring.

ACT will allow for a more seamless transition for residents within the health networks through data management. This will provide an efficient hospital discharge that allows for the initial doses to be dispensed at the hospital and the prescription data entered even prior to the resident’s discharge from the hospital.

340-B Funding

This aspect of ACT provides a unique ability to manage the required data tracking and reporting to adhere to 340-B regulatory requirements through managing the complete data of the ACO resident and individual dispensed dose inventory. Through the data, ACT is able to track each medication dispensed per resident, electronically manage inventory data to the dose level, and incorporate required resident demographics and clinical data to be able to administer the 340-B across the entire continuum of the ACO.

Clinical Data Management

The core of the ACT product is to effectively link a resident’s acute and LTC data, then to apply these to our decision support tools to identify clinical trends that will assist in maintaining the highest possible level of care while reducing the number of hospitalizations. ACT contains a repository of clinical support tools that will run resident clinical and demographic data against our data repository to provide actionable alerts around a number of healthcare identifiers such as wound care, appropriate diagnosis coding, lab and demographic data, and prescriptions. ACT can provide the alert allowing the facility to take action by informing the physician for medication or therapy change, scheduling a clinic visit, or for actively monitoring. The data allows the Facility to be more proactive in the care in partnership with entire ACO through ACT’s data integration. ACT allows for a seamless transition for residents within the ACO. Through data management, resident information is shared between the hospital and the facility. This allows healthcare providers a complete picture of resident history and provides a smooth hospital discharge allowing for the initial doses to be dispensed at the hospital and the prescription data entered even prior to the resident’s discharge from the hospital.
